We will be holding an on-site flu clinic this year for individuals age 7 or older on:
Sunday, October 8th from 9 am – 1 pm

St. Irenaeus parishioner Mat Szilard from Walgreens Pharmacy is delighted to offer the following immunizations:

•Flu Quadrivalent*
•Flu Quadrivalent High Dose (age 65+)*
•Pneumonia—Prevnar 20*
* all immunizations are preservative-free

We are requesting that interested individuals register for an appointment ahead of time.

1. Register online at https://wagsoutreach.com/ss/STI375502

2. Once you have completed the registration you will receive a confirmation email. In the email there will be a link on the right for “Vaccine Administration Record Form” (VAR). Please print the form (2 pages) complete the top section with your personal information and questions 1 thru 10. Also, please sign and date at the bottom. If you have trouble going online to print your “VAR” form, extras will be available in the gathering area of church.

3. Please bring the completed “Vaccine Administration Record Form” and your insurance card to the clinic.
